STEP 1: Trip Application and Release of Liability form.

A separate application is required from each adult guest.

Minors (under 18 years): Trip Application and Release of Liability - Minor Guest »
A parent or legal guardian may complete a single application for up to eight Minors. Guardians of Minors traveling to the Galapagos under the age of 8 are required to sign the Child Waiver—see below.

A clear copy of your passport’s entire photo page is required for all guests traveling outside the United States. You may send them to us via an email attachment or use a home scanner/copier to capture a clear image and use our secure server to upload them »
The image must capture all four corners of the page, be completely legible, and not be obscured by glare.

STEP 3: Insurance Declaration

We strongly encourage you to protect your investment in case circumstances beyond your control prevent you from traveling as planned, or interrupt your carefully planned arrangements. For U.S. residents, we have several companies we can recommend. Non-U.S. residents are free to purchase trip cancellation and interruption insurance independently from any licensed vendor.

Required waiver for children under the age of 8 on all Galapagos cruises

Please complete the following online Child Waiver Form »
While our minimum age is 6 years old, the Galapagos Islands are an expedition destination. You will be continuously exploring the islands with morning and afternoon shore visits for up to three hours at a time. Terrain can be challenging for smaller children. The sun may be strong or it could rain. There are no toilet facilities at any of the visitor sites on uninhabited islands. Guests are not allowed to walk back to the pickup point or remain along the shore by themselves, as the rules of the Galapagos National Park strictly state that guests must always be accompanied by a naturalist guide at all times. There are no daycare facilities aboard INTEGRITY and parents will be responsible for their children at all times, both aboard the yacht and on shore.
